I am looking for a developer to create a digital version i.e. an app of medical paper form for patients. The goal is to input data via a user-friendly interface (Web App or Windows App) and generate a PDF report at the end. The main goal is to make it user friendly but do not change the content. The next important step, is to make this app have a login feature to save the data entered for each patient into an exportable list of total patients.
